Click here to download a spreadsheet with documentation of clinics/meetings/tours that you have attended in recent years.  This information is very useful for documenting recertification points. 

The database is listed alphabetically by last name.  The number in a cell corresponds with a date of an ASQ Section 1118 event for a given individual.  A "1" means that you attended only one event on that particular date (eg. a tour or just the main presentation of a membership meeting).  A "2" means that you attended two events on that particular date (eg. in attendance during the clinic and the main presentation of a membership meeting).
